When do I need a sick / fit note?
7 days off sick or less
If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill. Instead they can ask you to confirm that you've been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. More information and the form for self-certification.
More than 7 days off sick
If you're off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Fit notes are sometimes referred to as medical statements or a doctor's note. Please call the surgery on 01642 210220 and selecting option 5 for general enquires (between 10.00am to 6.00pm) to arrange a routine telephone call to discuss your first sick note, please note an emergency appointment will not be booked for a sick note.
If your request is for a repeat sick note a task will be sent to the GP who will advise if a sick note can be issued or if another call is needed. The receptionist will ask you for some brief details and your request can take up to 72 hours. Please let the receptionist know if you would like it sending electronically to your phone or printed, please not if you are requesting a second paper copy a £10 fee will be charged.
